School Entry Health Examinations and Immunizations Against Communicable Diseases, Florida Statute 1003.22
School Entrance Physical Examination Requirements
Each student who is entitled to admittance to kindergarten, or is entitled to any other initial entrance into a Florida public or private school must present certification of a school entry physical examination performed within the twelve months before enrollment in school. Each District School Board and the governing authority of each private school, may establish a policy that permits a student up to 30 school days to present a certification of a school entry examination. A child may then be allowed to register and enter school. If the parent/guardian of the child fails to present evidence of a medical examination within thirty school days, the principal will exclude the student until examination documentation is presented to the school.
Immunization Requirements for Entrance
A child entering the school district of Walton County for the first time must present one of the four certificates below:
  1. DOH Form 680 (part A) Florida Certificate of Immunization for diphtheria/pertussis/tetanus (DPT), mumps/measles/rubella (MMR), poliomyelitis (OPV/IPV), the hepatitis B series, and varicella. Beginning with the 2008-2009 school year, students entering kindergarten will be required to have a second immunization for varicella. Each subsequent year thereafter, the next highest grade will also be included in the requirement. Beginning with the 2009-2010 school year, students entering seventh grade must have a Tdap booster (tetanus-diphtheria-pertussis containing vaccine instead of the previous tetanus-diphtheria Td) and each subsequent year thereafter, the next highest grade must be included in the requirement.
  2. DOH Form 680 (part B) Certificate of Immunization Temporary Medical Exemption or a written exemption (not to exceed thirty school days) issued by an authorized official to permit a child who transfers into the district to attend classes until his records can be obtained. At the end of the thirty-day exemption period, if the parent or the student fails to present a proper immunization certificate, the principal is to exclude the student until proper immunization documentation is presented to the school, or
  3. DOH Form 680 (part C) Certificate of Immunization Permanent Medical Exemption, or
  4. DOH Form 681 Certificate of Exemption For Religious Reasons
